Lost sign-off forms
It is your responsibility as students to ensure your evidence is uploaded by the deadline. Loss of paper forms will not be accepted as a valid reason for missing competencies. You are encouraged to upload forms at the earliest opportunity to avoid losing them in the first place; uploaded data is regularly backed up on the eFolio.
If you are in the unfortunate position of having lost a sign-off, your options are to print the form again and track down the original clinician to ask if they are able to recall the event and therefore to re-sign a duplicate form for you. If not, you will have to repeat it to get it signed off.
