Who can I ask to sign my eFolio sign off sheets?
Qualified health professional; Nurses, Theatre practitioners, HCAs etc. please ensure that they sign and print their name and also give their role and professional registration number (if they have one).
If a qualified health professional does not have a professional number (or they cannot remember it), please ask them to print as much information about their role/ ward / hospital/ Trust on your sign off sheet as possible. There needs to be sufficient information to make any sign offs traceable, if required. Inadequate detail will mean the competency cannot be signed off by the Faculty team.
